Job Description
ABOUT THE PROGRAM
Navigator is a program funded by state government which aims to support disengaged learners aged 10-17 years old to re-engage with an education. Jesuit Social Services delivers the Navigator Program in Hume Merri-bek.
The Navigator Program provides case management and outreach support to the most severely disengaged learners, to help create a pathway back into education. Navigator works one-on-one with young people and their families and schools to provide disengaged students with the specific and tailored support they need to reengage in education. Intake Assessment and Active Hold workers work collaboratively within a multidisciplinary team and also work closely with schools and key stakeholders to support each young person’s individualised pathway back to regular attendance at school and engagement in learning.
ABOUT THE ROLE
An exciting opportunity exists for a suitably experienced and qualified individual to take on an Intake Assessment and Active Hold role within the Hume Merri-bek team. The suitable candidate will have knowledge and/or experience working with young people experiencing mental illness, neurodiversity, disability; and those who may be part of the LGBTQIA+ community.
Intake Assessment and Active Hold workers are responsible for obtaining consent from the young person to participate, undertaking an initial assessment of the young person’s strengths, needs and barriers to educational engagement, developing rapport with the young person and family, providing specialist advice and/or referrals, assist with communication to schools and stakeholders, utilise assertive outreach techniques to engage hard to reach families, and triaging, whilst developing and maintaining relationships with stakeholders across a range of sectors.
The Navigator Hume Merri-bek team is based in Brunswick.
We have one full time (1FTE) position available with an attractive salary in the range of $85,126 - $91,587 pro rata, plus salary
Desired Skills and Experience
CANDIDATE REQUIREMENTS
Tertiary qualification/s and/or relevant experience in field
Demonstrated skills and experience in short-term (predominately phone-based) case management or case coordination
An understanding of adolescent development and the factors that can contribute to a young person’s disengagement from learning.
High degree of cultural competency and understanding of culturally appropriate service delivery in relation to Aboriginal young people, their families and communities and CALD young people, their families and communities.
Superior written and verbal (specifically email and phone-based) communication skills, including the capacity to liaise with a range of stakeholders both government and non-government.
